Job Overview

Homerton Healthcare NHS Foundation Trust is seeking a highly skilled and dedicated Children's Community Health Specialist to join our Community Children's Nursing team. As a key member of our team, you will provide specialist nursing care to children in the community, working closely with parents and carers to ensure the best possible outcomes for our young patients.

Main Duties
  • Provide high-quality nursing care to children with varying health needs, working autonomously and as part of a team.
  • Develop and implement individualised care plans, taking into account the unique needs and circumstances of each child and their family.
  • Work closely with other healthcare professionals, including doctors, therapists, and social workers, to ensure seamless care and support for children and their families.
  • Conduct assessments, deliver care, and evaluate the effectiveness of care plans, making adjustments as needed to ensure the best possible outcomes for children.
  • Manage client caseloads, prioritising tasks and delegating responsibilities as necessary to ensure efficient and effective care.
  • Communicate complex and sensitive information to children, parents, and carers, using clear and concise language to ensure understanding and engagement.
