About the Role

We are seeking an experienced and compassionate Healthcare Assistant to join our team in providing end of life care services to patients in their homes. As a key member of our community care team, you will work closely with our Macmillan Nurses, District Nurses, and other healthcare professionals to deliver high-quality, person-centered care.

Key Responsibilities
  • Assist patients with their personal care needs, including bathing, dressing, and medication management.
  • Provide emotional support and companionship to patients and their families.
  • Work collaboratively with our community nursing team to develop and implement individualized care plans.
  • Participate in ongoing training and professional development to enhance your skills and knowledge in end of life care.
  • Contribute to the maintenance of accurate and up-to-date patient records.
Requirements
  • NVQ level 3 health and social care qualification.
  • At least 6 months of experience working in the NHS community.
  • Valid driving license with less than 6 points.
  • Ability to work flexibly, including evenings and weekends.
