About the Role

We are seeking a highly skilled and experienced Specialist Practitioner to join our team at Lancashire & South Cumbria NHS Foundation Trust. As a Specialist Practitioner, you will play a vital role in providing support to individuals before they leave prison, promoting their independent self-management of healthcare needs.

Key Responsibilities
  • Assess and refer individuals to relevant services, utilizing your expertise in trauma-informed working and knowledge of validated assessment tools.
  • Provide clinical oversight to the caseloads of other staff within the team, ensuring high-quality care and support.
  • Work collaboratively with individuals, carers, families, and other service providers to gather pertinent information and promote self-management of healthcare needs.
Requirements
  • Registered Healthcare Practitioner, Registered Mental Health Nurse (RMN), Registered Nurse, Learning Disability, or Social Worker.
  • Degree qualification in a relevant subject or demonstrable evidence of equivalent experience.
  • Post-qualifying experience within their field of specialism or 12 months relevant experience in a community, criminal justice, or secure setting.
