We are seeking a highly motivated and compassionate Lung Oncology Support Worker to join our team at East Suffolk and North Essex NHS Foundation Trust. As a key member of our Lung Oncology team, you will play a vital role in providing exceptional patient care and support to individuals diagnosed with lung cancer.
Key Responsibilities- Provide holistic needs assessments and care planning for patients, ensuring their individual needs are met and they are aware of available wellbeing services and support.
- Work closely with Clinical Nurse Specialists (CNS) to support patients and ensure their care is personalized.
- Respond to patient telephone enquiries, triage calls, and escalate to the CNS as necessary.
- Provide administrative support to the Lung Oncology CNS team, including data collection and analysis.
- Develop and maintain effective relationships with patients, families, and healthcare professionals to ensure seamless care coordination.
- Contribute to the development and implementation of patient-centered care plans, ensuring timely and effective interventions.
- Currently employed in a relevant healthcare or social care background, with experience in patient care delivery.
- Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
- Basic computer skills and experience with electronic patient records.
- Ability to study to level 3 or equivalent, with a strong foundation in maths and English.
