Job Title: Senior Community Nurse

We are seeking an experienced and skilled Senior Community Nurse to join our team at Cambridgeshire and Peterborough NHS Foundation Trust. As a Senior Community Nurse, you will play a vital role in delivering high-quality care to individuals with learning disabilities.

Key Responsibilities:
  • Manage a caseload of clients with complex learning disabilities, providing assessment, identification, and treatment of their health needs.
  • Contribute to the planning, development, and evaluation of learning disability services, including participating in defined projects as delegated by the LDP management team.
  • Work autonomously to manage a specialist caseload, providing assessment, identification, and treatment of the health needs of adults with learning disabilities.
  • Develop and implement integrated individual support plans for each client in conjunction with team members as appropriate.
  • Input, analyze, and present data/statistics to inform evidence-based practice.
  • Write timely professional reports/records and action plans in line with professional guidelines that comply with CPFT and County Council record-keeping policies.
Requirements:
  • RNLD registration with the Nursing and Midwifery Council.
  • Registered Practice Educator or mentor in line with Nursing and Midwifery Council requirements.
  • Experience of working with adults with learning disabilities, managing a caseload, and nursing assessment and intervention relevant to adults with learning disabilities.
Desirable Criteria:
  • Experience of supervising staff.
  • Additional clinical training relevant to the specialism.
  • Post-qualifying experience within a community MDT.
  • Care coordination in CPA/case management.
  • Experience in providing clinical supervision.
Skills and Abilities:
  • Effective written and verbal communication skills.
  • Effective organization and time management skills.
  • Ability to reflect and critically appraise own performance and of those you supervise.
  • Ability to prioritize your own work and the work of those you supervise.
  • Ability to work under pressure and cope with stress.

Knowledge and Understanding:
  • Mental Health Act and Mental Capacity Act knowledge.
  • IT skills.
  • Understanding of research and audit processes.
  • Understanding of clinical governance.
  • Awareness of current trends, legislation, and guidance in support for people with learning disabilities and their carers.
  • Knowledge of treatment strategies.
  • Knowledge of the Enabling Approach to support people to meet their full potential.
  • Evidence of continuous professional development.
