About the Role

We are seeking an experienced and dynamic Perinatal Outreach Practitioner to join our Specialist Perinatal Outreach Team. The successful candidate will provide outreach services across the Greater Manchester area, working with individuals in the perinatal period to bridge the gap between inpatient and community services.

Key Responsibilities
  • Assess, formulate, and prescribe care planning, including discharge planning within agreed protocols.
  • Provide support to complex cases, ensuring the best care for mother/birthing person and baby.
  • Work closely with the ward and community multidisciplinary team to identify patients who would benefit from outreach services.
  • Manage and triage referrals into the ward and outreach team.
  • Act as bed manager for Andersen Ward Mother and Baby Unit.
