Perinatal Outreach Practitioner

We are seeking an experienced and dynamic Perinatal Outreach Practitioner to join our Specialist Perinatal Outreach Team. The successful candidate will work closely with the ward and community multidisciplinary team to identify patients who would benefit from our outreach services.
Key Responsibilities- Perform home visits, often on a lone working basis, to assess and support patients in the perinatal period.
- Manage and triage referrals into the ward and outreach team, ensuring timely and effective support for patients.
- Act as bed manager for Andersen Ward Mother and Baby Unit, coordinating care and ensuring safe discharge for patients.
- Work closely with mothers, birthing people, and their families to provide ongoing support and facilitate safe discharge from the ward.
- Support patients in the community who are vulnerable to admission to the MBU, providing short-term interventions and working with allocated care coordinators.
- Experience of working with people in the perinatal period, ideally in a community or ward mental health setting.
- Passion for working with people in the perinatal period, with a commitment to delivering high-quality care and support.
- A competitive salary, with 30% additional pay for evenings and Saturdays, and 60% additional pay for Sundays and Bank Holidays.
