Job Summary

Greater Manchester Mental Health NHSFT is seeking an experienced and dynamic individual to join our Perinatal Outreach Team as a Perinatal Outreach Practitioner. This exciting new role will bridge the gap between inpatient and community perinatal services, providing outreach support to individuals in the perinatal period.

Main Responsibilities
  • Assess and formulate care plans for individuals in the perinatal period, including discharge planning within agreed protocols.
  • Provide support to complex cases, ensuring that both mother/birthing person and baby receive the best care.
  • Work closely with the ward and community multidisciplinary team to identify patients who would benefit from outreach support.
  • Manage and triage referrals into the ward and outreach team, acting as bed manager for Andersen Ward Mother and Baby Unit.
