Job Summary

Main Responsibilities

As a Community Nurse, your key responsibilities will include:

  • Providing comprehensive assessments and care planning for patients with complex health needs
  • Delivering high-quality nursing care to patients in their own homes and communities
  • Working collaboratively with multidisciplinary teams to ensure seamless care delivery
  • Developing and maintaining strong relationships with patients, families, and carers
  • Participating in education and training to enhance your skills and knowledge

Requirements

To be successful in this role, you will need to have:

  • Current UK NMC registration as a Registered Nurse
  • Recent clinical nursing experience negotiating and liaising within a multi-disciplinary team
  • Experience of working with patients with complex health needs
  • Excellent communication and interpersonal skills
  • A commitment to ongoing learning and professional development
