About the RoleThis is a full-time permanent position, and you will be working closely with our team to develop and implement strategies to improve patient outcomes and experiences. You will be responsible for managing a defined caseload, ensuring accurate nursing records, and participating in public health initiatives. You will also be expected to work collaboratively with other teams within Adult Community Services and other providers to ensure optimum patient-centered care.
Key Responsibilities- Provide leadership and management to a team of community nurses and other staff

- Undertake non-medical prescribing and maintain own competence in this area
- Follow Trust procedures concerning vulnerable Adults and Children
We are looking for a Registered Nurse with experience in management and leadership, as well as a strong understanding of recent developments in the NHS and evidence-based practice. You should have a Specialist Nursing Practice (District Nursing) Degree or be willing to undertake training, as well as Nurse Prescribing or be willing to undertake training. You should also have a proven track record of leadership and management, as well as experience of teaching others and participating in clinical audits.
