Job Summary

Central London Community Health Trust (CLCH) is seeking a highly skilled and experienced Deputy Team Lead to join our Adult Community Services team in Hertfordshire. As a key member of our team, you will be responsible for providing leadership and management to a team of community nurses and other staff, ensuring the delivery of high-quality patient care and services.

About the Role

This is a full-time permanent position, and you will be based in Hertfordshire. As Deputy Team Lead, you will be responsible for:

  • Providing leadership and management to a team of community nurses and other staff
  • Ensuring the delivery of high-quality patient care and services
  • Coordinating and supervising the nursing team and learners as required
  • Participating in the long-term management of patients within the locality
  • Assisting with the management of long-term conditions to a defined population
Key Responsibilities

As Deputy Team Lead, you will be responsible for:

  • Ensuring a team philosophy that embraces a holistic approach to patients, carers, and relatives, ensuring all are treated with respect for their privacy and dignity
  • Promoting optimum standards of health through education and encouraging patients to adopt a self-care management model
  • Working with hospital discharge teams to raise concerns of patients admitted to or discharged from acute hospitals, including those with complex care needs
  • Ensuring the team promotes good health and participates in public health initiatives
  • Undertaking non-medical prescribing and maintaining own competence in this area
Person Specification

We are looking for a candidate with:

  • Registered Nurse qualification
  • Practice Supervisor/Assessor or equivalent experience
  • Specialist Nursing Practice (District Nursing) Degree or willingness to undertake training
  • Nurse Prescribing or willingness to undertake training
  • Knowledge of recent developments in the NHS and evidence-based practice
  • Proven leadership and management experience, including team management and conflict resolution
