Job Summary

As a Community Nurse, you will play a vital role in providing high-quality care to patients in their own homes, promoting independence and improving health outcomes.

Main Responsibilities

As a Community Nurse, your key responsibilities will include:

  • Conducting comprehensive assessments of patients' needs and developing personalized care plans
  • Providing wound care, palliative care, and IV therapies to patients in their own homes
  • Working collaboratively with multidisciplinary teams to ensure seamless care delivery
  • Supporting patients with complex health needs, including those with diabetes and long-term conditions
  • Participating in audit and research projects to improve service delivery and patient outcomes
Requirements

To be successful in this role, you will need:

  • Current UK NMC registration as a Registered Nurse
  • Recent clinical nursing experience, preferably in a community setting
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
  • Knowledge of PREP, infection control, and general wound care
  • A full, valid UK driving license and access to a car for business purposes
