We are seeking a highly skilled and experienced Community Nursing Sister to join our team at Berkshire Healthcare Foundation Trust. As a key member of our District Nursing team, you will play a pivotal role in managing a nursing team and a caseload, ensuring the delivery of a seamless service to patients.
Key Responsibilities- Operate within a Case Management framework and coordinate care for an identified caseload of patients.
- Assess patient need and plan, implement, and evaluate programs of Nursing Care.
- Identify and plan preventative measures and anticipatory care needs.
- Proactively monitor patients following review dates set out in care plans.
- Assess patients with complex clinical needs using clinical examination skills, history taking, and evaluation skills.
- Manage End of Life Care and work in partnership with health, social, and voluntary agencies to ensure patients are treated with dignity and respect and to support their families and carers.
- Prepare patients and their families/carers for changes in patients' conditions and support choice for end-of-life care.
- Complete Management 1-1s with your team, as well as appraisals, and ensure staff are attending clinical supervision.
- Submit statistical and contractual monitoring returns within agreed timescales.
- Ensure Mandatory and Statutory training is booked and completed within time scales for yourself and your team.
- Book and take annual leave as laid down in Trust policy and Standard Operating Procedures, ensuring continuity of service by ensuring your team also take regular annual leave.
