Key Responsibilities:
- Manage a diverse caseload, working to prevent hospital admissions and support patients in their preferred place of care.
- Maintain accurate and confidential patient records, adhering to Trust policies.
- Lead an effective team, providing clinical advice and support to staff and patients.
- Manage a defined caseload, assessing care needs, developing and implementing care programs, and evaluating their effectiveness.
- Accountable for the standards of care and the activity of your team, acting as a role model and motivator for staff.

Previous Experience:
