Job Title: Community Nursing Sister

We are seeking an experienced Community Nursing Sister to join our team within the Wokingham Community Nursing Services. As a Community Nursing Sister, you will be responsible for leading a team of nurses in providing high-quality care to patients in their own homes.

About the Role

The successful candidate will have a strong background in community nursing, with experience in leading teams and managing complex caseloads. You will be responsible for ensuring the delivery of high-quality, evidence-based care to patients, their families, and carers in the community.

Main Duties
  • Responsible for a complex and diverse caseload, actively working to prevent admission to the acute sector and supporting patients in their preferred place of care.
  • Responsible for maintaining detailed, accurate and confidential patient records, adhering to Trust policies.
  • Responsible for the management of a defined caseload of patients, with continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of care programmers.
  • Accountable for the standards of care and for determining the activity of your team.

Requirements

To be considered for this role, you will need:

  • Adult nursing qualification along with NMC registration.
  • A desire to develop leadership and management skills.
  • A passion for community nursing, that matches our passion to provide excellent healthcare for our patients within their own home.
  • You will need to either have access to a vehicle which can be used for work purposes.
