Clinical Case Manager Job Description

We are seeking a skilled Clinical Case Manager to join our team at HCA Healthcare UK. As a Clinical Case Manager, you will play a crucial role in assessing, planning, implementing, and evaluating care options and services that meet the patients' health needs.

Key Responsibilities:

  1. Work closely with healthcare professionals to assess, plan, implement, and evaluate care options and services.
  2. Act as a liaison between patients, hospitals, physicians, and third-party payers to optimize patients' community treatment and services.
  3. Develop and implement comprehensive discharge plans to ensure seamless transitions of care.
  4. Collaborate with multidisciplinary teams to provide patient-centered care and achieve optimal outcomes.
  5. Utilize expertise in utilization review and discharge planning to optimize resource allocation and reduce healthcare costs.

Requirements:

  1. NMC Registered Nurse with substantial postgraduate experience in adult and paediatric nursing.
  2. Experience in utilization review and extensive discharge planning experience essential.
  3. Experience as a Senior Staff Nurse or above essential.
  4. Paediatric experience and knowledge of discharge planning and community resources.
  5. Excellent communication, organization, and computer skills, with a dynamic approach to problem-solving.
