Job Overview

An exciting opportunity has arisen to join our dedicated team of healthcare professionals working with looked after children and their families in North Wales. Our team is committed to providing high-quality services that support the health, development, and emotional well-being of children in care.

Main Responsibilities

The successful candidate will work closely with our Looked After Children's Nursing Team to coordinate the completion and implementation of statutory health assessments and care plans for looked after children. This will involve working in partnership with multi-disciplinary teams, including school nursing, health visiting, community paediatricians, and other healthcare professionals to ensure the best possible health and social outcomes for children in care.

Key Accountabilities

The post holder will be responsible for:

  • Coordinating the completion and implementation of statutory health assessments and care plans for looked after children
  • Working in partnership with multi-disciplinary teams to ensure the best possible health and social outcomes for children in care
  • Providing professional leadership and guidance to colleagues
  • Developing and maintaining effective relationships with children, families, and carers

Requirements

The ideal candidate will have:

  • Professional experience working with looked after children and their families
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Strong analytical and problem-solving skills
  • Desirable: ability to speak Welsh
