We are seeking a highly skilled and experienced Registered Mental Health Nurse to join our Specialist Dementia and Frailty Team as a Nurse Practitioner. The successful candidate will be responsible for providing high-quality, evidence-based care to patients with dementia and frailty, working closely with the Advanced Nurse Practitioner and other healthcare professionals.
Key Responsibilities- Conduct comprehensive assessments and reviews of care for patients with dementia and frailty, in line with mental health care clusters.
- Develop and implement individualized care plans, ensuring the best possible outcomes for patients.
- Supervise and manage junior team members, providing guidance and support as needed.
- Collaborate with other healthcare professionals to ensure seamless care delivery and effective communication.
- Participate in performance management and supervision of lower-banded staff, promoting a culture of excellence and continuous improvement.
- Registered Mental Health Nurse (RMN) qualification.
- Experience in performance management and supervision of lower-banded staff.
- Understanding of health legislation, principles of CPA and Care Management, Mental Capacity Act, Mental Health Act 1983, and safeguarding.
- Full UK driving license and access to a car for work purposes.
