About the Role

We are seeking a highly skilled and dedicated Registered Mental Health Nurse (RMN) to join our Specialist Dementia and Frailty Team as a Nurse Practitioner. As a key member of our multidisciplinary team, you will be responsible for providing high-quality, evidence-based care to patients with dementia and frailty.

Key Responsibilities
  • Conduct comprehensive assessments and develop individualized care plans in collaboration with patients, carers, and other healthcare professionals.
  • Provide expert care and support to patients with dementia and frailty, including medication management, wound care, and palliative care.
  • Supervise and mentor junior staff members, promoting a culture of excellence and continuous learning.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Participate in quality improvement initiatives and contribute to the development of best practices within the team.
Requirements
  • Registered Mental Health Nurse (RMN) qualification and current registration with the NMC.
  • Experience in dementia and frailty care, with a strong understanding of relevant legislation and guidelines.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and healthcare professionals.
  • Ability to work autonomously and as part of a team, with a flexible approach to work and a willingness to travel across sites.
  • Current driving license and access to a car for work purposes.
