Job Title: Registered Mental Health Nurse

We are seeking a highly skilled and compassionate Registered Mental Health Nurse to join our team at the Transformation Unit. As a key member of our community health team, you will play a vital role in providing high-quality care to patients with dementia and frailty.

About the Role

As a Registered Mental Health Nurse, you will be responsible for assessing, planning, implementing, and evaluating packages of care for patients with dementia and frailty. You will work closely with the Advanced Nurse Practitioner to ensure that patients receive comprehensive and evidence-based care.

You will also have the opportunity to supervise and manage junior team members, providing guidance and support to help them develop their skills and knowledge.

Key Responsibilities
  • Conduct comprehensive assessments and reviews of care for patients with dementia and frailty
  • Develop and implement individualized care plans that meet the unique needs of each patient
  • Work collaboratively with the Advanced Nurse Practitioner to ensure that patients receive high-quality care
  • Supervise and manage junior team members, providing guidance and support to help them develop their skills and knowledge
  • Participate in multidisciplinary team meetings to discuss patient care and develop strategies for improving outcomes
Requirements
  • Registered Mental Health Nurse (RMN) qualification
  • Experience in the performance management and supervision of lower-banded staff
  • Knowledge of health legislation, principles of CPA and Care Management, Mental Capacity Act, Mental Health Act 1983, and safeguarding
  • Full UK driving license and access to a car for work purposes
