Nurse Practitioner

We are seeking a highly skilled and dedicated Nurse Practitioner to join our Specialist Dementia and Frailty Team. As a key member of our multidisciplinary team, you will be responsible for providing high-quality, evidence-based care to patients with dementia and frailty.
Key Responsibilities- Conduct comprehensive assessments and develop individualized care plans in collaboration with patients, families, and healthcare professionals.
- Provide expert care and support to patients with dementia and frailty, including medication management, wound care, and palliative care.
- Supervise and mentor junior team members, promoting a culture of excellence and continuous learning.
- Collaborate with community partners, service users, and carers to ensure seamless care transitions and optimal outcomes.
- Participate in quality improvement initiatives and contribute to the development of best practices in dementia and frailty care.
- Registered Mental Health Nurse (RMN) with a strong background in dementia and frailty care.
- Experience in performance management and supervision of lower-banded staff.
- Knowledge of health legislation, principles of CPA and Care Management, Mental Capacity Act, Mental Health Act 1983, and safeguarding.
- Full UK driving license and access to a car for work purposes.
