We are seeking an experienced Advanced Clinical Practitioner to join our East Kent HCP Frailty/Home Treatment Service. As a key member of our team, you will provide high-quality clinical care to older adults in various healthcare settings.

Key Responsibilities
  • Apply advanced clinical expertise to assess, diagnose, treat, and manage complex patient needs.
  • Lead and monitor quality improvements through clinical audit, evidence-based practice, and clinical supervision.
  • Develop and implement non-medical led models of care across emergency departments, inpatient wards, and community teams.
  • Contribute to the development of clinical guidelines and evidence-based practice.
