Job Summary

An exciting opportunity has arisen for a registered nurse or allied health professional to join the Clinical Review team within the NHS Bristol, North Somerset and South Gloucestershire Integrated Care Board.

The successful candidate will work with our system partners and care providers to support the functions of the NHS Care (Education) & Treatment review policy and the Transforming care programme.

Main Responsibilities
  • Collaborate with clinical and non-clinical staff to ensure seamless care coordination for individuals with learning disabilities and/or Autism.
  • Develop and implement effective care plans to reduce hospital admissions and improve community-based care.
  • Conduct regular reviews and assessments to identify areas for improvement and develop strategies to address them.
  • Work closely with our system partners to ensure that individuals receive safe and appropriate care.
  • Provide training and support to care providers to enhance their knowledge and skills in supporting individuals with complex needs.

Requirements
  • Registered nurse or allied health professional with experience of working with individuals with learning disabilities and/or Autism.
  • Strong communication and administrative skills, with the ability to work effectively in a politically sensitive environment.
  • Experience of managing risks and reporting, with a sound knowledge of the Mental Health Act and the Mental Capacity Act.
  • Ability to analyse complex issues and make informed decisions in the absence of required information.
  • Experience of working collaboratively with other agencies and organisations to deliver high-quality care.
