An exceptional opportunity has emerged for a motivated and innovative Band 6 Community Rehabilitation Physiotherapist to become part of the Somerset NHS Foundation Trust. This role focuses on enhancing the quality of life for individuals within the community, promoting independence and well-being.

Main Responsibilities

The primary objective of the Community Rehabilitation team is to assist patients in remaining at home, facilitating recovery, and re-engaging with vital aspects of their lives while preventing unnecessary hospital admissions. Key responsibilities include:

  • Delivering high-quality, patient-centered care and specialized rehabilitation to adults referred to the Community Rehabilitation Service.
  • Providing expert guidance on rehabilitation management to patients, families, caregivers, and other multidisciplinary team members.
  • Collaborating effectively within a multidisciplinary team to support patients in achieving their rehabilitation goals.
  • Overseeing the operations of outpatient clinics, including those focused on neurological conditions and falls prevention.

Qualifications and Experience

Essential:

  • Degree or Master's in Physiotherapy.
  • HCPC Registration.
  • Demonstrated commitment to continuous professional development.
  • Proven postgraduate experience in relevant clinical settings.
  • Experience in a multidisciplinary team and community-based practice.
  • Valid UK driving license and access to a vehicle.

Desirable:

  • Membership in the Chartered Society of Physiotherapy.
  • Experience in service development and review.
