We are seeking a motivated and skilled Community Rehabilitation Physiotherapist to join our dynamic team at Somerset NHS Foundation Trust. This role is pivotal in providing high-quality rehabilitation services to individuals within the community, ensuring they can achieve their personal health goals and maintain independence.
Key Responsibilities
The primary objective of the Community Rehabilitation team is to assist patients in remaining at home, facilitating recovery, and engaging with essential life activities while preventing unnecessary hospital admissions. Your responsibilities will include:
- Delivering patient-centered care and specialized rehabilitation services to adults referred to the Community Rehabilitation Service.
- Offering expert guidance on rehabilitation strategies to patients, families, caregivers, and multidisciplinary team members.
- Collaborating effectively within a multidisciplinary team to support patients in reaching their rehabilitation goals.
- Overseeing the operation of outpatient clinics, including those focused on neurological conditions and falls prevention.

Qualifications and Experience
Essential Qualifications:
- A recognized Diploma, Degree, or Master's in Physiotherapy.
- Registration with the Health and Care Professions Council (HCPC).
- A portfolio demonstrating ongoing professional development, including participation in recent postgraduate courses.
Desirable Qualifications:
- Membership in the Chartered Society of Physiotherapy.
- Involvement in relevant special interest groups.
- Completion of postgraduate courses in elderly care or rehabilitation.
Essential Experience:
- Proven post-graduate experience with a strong clinical background relevant to the role.
- Experience in various clinical rehabilitation settings, particularly with elderly patients facing complex health challenges.
- Demonstrated ability to work effectively as part of a multidisciplinary team.
- Experience in community-based practice.
- Familiarity with clinical governance principles.

- A valid UK driving license and access to a vehicle.
