Job Summary

We are seeking a skilled Rehabilitation and Wellbeing Specialist to join our team at Second Step. As a key member of our community rehabilitation service, you will play a vital role in supporting individuals with long-term mental health challenges to achieve a well-rounded and lasting recovery.

Key Responsibilities

  • Work collaboratively with a multi-agency team to provide holistic support to individuals, promoting self-management skills, informed decision-making, and independence.
  • Utilize a psychologically informed approach to deliver recovery and crisis management tools, empowering individuals to access community resources and services.
  • Develop and maintain strong relationships with service users, families, and community stakeholders to ensure seamless support and coordination.
  • Contribute to the development and implementation of individualized recovery plans, aligning with the needs and goals of each service user.
  • Participate in regular team meetings and contribute to the ongoing evaluation and improvement of our community rehabilitation service.

Work Environment

You will work across various venues, including office spaces and community settings, with some home-based working. Our service operates 7 days a week, with a 4-week rolling rota, ensuring comprehensive support for our service users.

Requirements

  • Ability to travel to multiple appointments daily, including in rural areas with limited public transport.
  • Flexibility to work a 0.4FTE/15hrs per week schedule, spread across 2 days, to align with our existing rota.
