About the Role

We are seeking an experienced and ambitious leader to join our Community Team Leader group in our Planned Care community team in North Bradford localities. As a Community Team Leader, you will provide leadership and take managerial and supervisory responsibility for a group of clinical practitioners working as a multidisciplinary team providing adult physical health services in the community.

The successful candidate will have a strong background in community nursing and experience in leadership and management. You will work closely with Operational Managers and Clinical Team leaders to deliver on our transformational plans to implement the NHS plan, including developing our Planned and Unplanned services and supporting a more integrated model of care for our Specialist services.

Key Responsibilities
  1. Provide leadership and managerial support to a team of clinical practitioners.
  2. Develop and implement plans to improve the quality of care and services provided in the community.
  3. Work closely with Operational Managers and Clinical Team leaders to deliver on transformational plans.
  4. Support the development of a more integrated model of care for Specialist services.
  5. Collaborate with other health and social care professionals to provide proactive and safe care to our local population.
