Job Summary:

We are seeking a skilled and compassionate Healthcare Liaison Specialist to join our team at the NHS. As a Physical Health Support Practitioner, you will play a vital role in supporting individuals with learning disabilities who have physical healthcare needs.

Key Responsibilities:

  • Provide expert advice and support to clinicians within the NHS and beyond to ensure that individuals with learning disabilities receive high-quality physical healthcare.
  • Work collaboratively with healthcare professionals to develop and implement care plans that meet the unique needs of individuals with learning disabilities.
  • Support individuals with learning disabilities to access healthcare services and navigate the healthcare system.
  • Provide management and clinical supervision to peers and junior staff, ensuring that they receive the support and guidance they need to deliver high-quality care.
  • Manage a complex caseload, ensuring that records are accurate and up-to-date, and that individuals receive timely and effective support.

Requirements:

  • Strong knowledge of healthcare policies and procedures, particularly those related to learning disabilities.
  • Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong focus on collaboration and partnership working.
  • Strong problem-solving and analytical skills, with the ability to think critically and creatively.
  • Ability to manage a complex caseload and prioritize tasks effectively.
