Job Title: Adult Advanced Practitioner

Local Care Direct is a non-profit social enterprise operating 24/7 to provide primary and urgent healthcare services across West Yorkshire.

About the Role

We are seeking a qualified Adult Advanced Practitioner to join our team at King Street Walk-In Centre in Wakefield. As a key member of our healthcare team, you will provide high-quality care to patients with primary and urgent healthcare needs.

Key Responsibilities
  • Undertake assessment, diagnosis, treatment, and referral of patients with primary and urgent healthcare needs.
  • Provide clinical mentorship and leadership to trainee and peer practitioners.
  • Participate in audit, data collection, and activity monitoring to improve performance and inform future service developments.
  • Report patient safety incidents and contribute to quality improvement initiatives.
  • Develop and maintain clinical skills and competencies through education and training.
Requirements
  • Current registration with HCPC, NMC, or GPhC.
  • Experience of working in primary and urgent care.
  • Ability to pass pre-employment health assessment.
  • Up-to-date knowledge of health agenda and health inequalities.
