Position Overview

As a Community Care Specialist, you will play a vital role in the delivery of social services within the community. This position involves working collaboratively with a multidisciplinary team to ensure that individuals receive the necessary support and care tailored to their unique needs.

Key Responsibilities

  • Conduct assessments to determine the care and support needs of individuals within the community.
  • Develop and review outcome-focused care plans in partnership with clients, their families, and caregivers.
  • Collaborate with various health and social care professionals, including nursing, occupational therapy, and public health teams.
  • Maintain effective communication with all stakeholders to ensure seamless service delivery.
  • Stay informed about relevant legislation and best practices in social care.

Essential Qualifications

A recognized qualification in social work or equivalent is required, along with registration with the appropriate professional body. Experience in health or social work settings is essential, particularly in assessment and care management.

Skills and Attributes

The ideal candidate will possess strong communication, analytical, and organizational skills. You should be capable of working independently and managing your workload effectively. A solid understanding of the challenges faced by individuals with disabilities, chronic illnesses, or age-related issues is crucial.
