About the Role

We are seeking a dedicated and compassionate Care and Support Professional to join our team at Cygnet Gledholt. As a Care and Support Professional, you will play a vital role in providing high-quality care and support to individuals with learning disabilities, empowering them to live fulfilling lives.

Key Responsibilities

  • Provide person-centred care and support to individuals with learning disabilities, promoting their independence and well-being.
  • Work collaboratively with the care team to develop and implement individualised care plans, ensuring the needs of each person are met.
  • Assist with daily living tasks, such as personal care, meal preparation, and household chores.
  • Support individuals to engage in activities that promote their physical and emotional well-being, such as exercise, hobbies, and social events.
  • Monitor and report any changes in an individual's condition, ensuring their safety and well-being.
