Key Responsibilities

PRIMARY OBJECTIVE To assist Therapy professionals in delivering high-quality, holistic, person-centered care and support to clients, enabling them to achieve their rehabilitation goals.

CORE DUTIES AND RESPONSIBILITIES

- Collaborate with a multi-disciplinary team to ensure that the cognitive, social, emotional, physical, and functional needs of clients are effectively met.

- Provide support to clients facing cancer or other life-limiting conditions, particularly those with specialized palliative or neurological requirements.

- Actively promote and uphold the individual rights and choices of clients, ensuring their beliefs and preferences are respected.

- Report any changes in treatment status or client condition to the appropriate Therapist promptly and assist in progressing treatment as directed.

- When necessary, review, order, and fit equipment, demonstrating its safe use to clients and their families, whether in the facility or at home.

- Liaise with clients and their families to assist in completing forms, gathering information, and coordinating interventions.

- Support the Therapy Team by performing administrative tasks, such as collecting referrals and recording monthly activity data.

- Collect, compile, evaluate, and report information, maintaining accurate records of client information in both written and electronic formats.

- Ensure the safe and efficient operation of the Service, maintaining a secure working environment, upholding client confidentiality, and occasionally undertaking reasonable additional tasks.
