Shropshire Community Health NHS Trust is seeking a highly skilled and motivated Community Staff Nurse to join our South West Neighbourhood Team. As a Community Staff Nurse, you will play a vital role in delivering high-quality patient care and supporting the delivery of excellent patient outcomes.

Key Responsibilities

  • Provide holistic assessment and care to patients in their own homes or chosen place of residence.
  • Develop, implement, and evaluate care plans in collaboration with other healthcare professionals.
  • Work autonomously and as part of a multidisciplinary team to promote independence and timely discharge.
  • Contribute to the development and implementation of evidence-based practice.

Requirements

  • RGN NMC Registered.
  • Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes.
  • Post-registration general experience.
  • Experience of working in a multi-disciplinary/multi-agency health/social care environment.
  • Experience and knowledge of basic administration systems, including Microsoft Office.
  • Experience of successful partnership working arrangements with other services.
  • Previous community experience.
