Occupational Therapy Care Manager

About the Role:

We are seeking an experienced Occupational Therapy Care Manager to join our Community Independence Service (CIS) team. As an Occupational Therapy Care Manager, you will play a key role in facilitating early supported discharge from hospital by delivering skilled Occupational Therapy interventions to individuals in their own homes.

Key Responsibilities:

  • Deliver goal-oriented plans to promote health and independence
  • Manage a caseload of clients and work closely with Reablement care providers
  • Implement moving and handling plans and assist with hospital discharges
  • Demonstrate and instruct service users and their family or carers on equipment use and maintenance
  • Identify safeguarding enquiries and concerns and work in partnership with statutory agencies

About You:

We are looking for an individual with a degree or equivalent in Occupational Therapy, HCPC registration, and experience working in a multi-disciplinary setting with adults with disabilities. You will have practical experience of completing complex movement and handling assessments and support plans, as well as expertise in exploring equipment options to enable individuals to be safe and independent.
