Position Overview

The Liverpool Heart and Chest Hospital is dedicated to delivering an exceptional pharmacy service to the palliative care sector. We are looking for a dedicated and knowledgeable Lead Clinical Pharmacist to enhance our pharmacy team focused on palliative care. In this essential position, you will collaborate with a committed multidisciplinary team (MDT) to provide specialized pharmaceutical care to individuals facing life-limiting conditions.

As a Lead Clinical Pharmacist, your role will be vital in improving medication management, ensuring patient safety, and enhancing the quality of life for those in our care. Your expertise will be crucial in formulating and executing evidence-based pharmaceutical care strategies, working alongside healthcare professionals, and offering comprehensive medication education to patients and their families.

You will also have the opportunity to engage with the broader pharmacy team at Liverpool Heart and Chest Hospital and take on clinical responsibilities within a specialized tertiary trust.

Main Responsibilities

1. Deliver and support high-quality pharmaceutical care tailored to the needs of palliative care patients.

2. Collaborate with palliative care practitioners and other professionals to facilitate the implementation of national and local health policies.

3. Engage in educational and training initiatives for healthcare professionals and students in palliative care.

Key Duties

1. Conduct routine assessments of prescriptions for legality, safety, and appropriateness in accordance with relevant regulations.

2. Develop and implement local prescribing policies and formulary recommendations.

3. Formulate individualized pharmaceutical care plans and adjust them based on patient responses and clinical outcomes.

4. Provide pharmaceutical advice to patients and their caregivers in various situations, addressing complex conditions.

5. Advise medical and nursing staff on drug selection, dosage, administration routes, and potential interactions.

6. Monitor and manage drug expenditure to ensure cost-effective medication use.

7. Record and report adverse drug reactions as necessary.

8. Participate in the provision of pharmaceutical services during out-of-hours and weekends.

9. Attend multidisciplinary ward rounds and provide clinical support to all patients.

10. Engage in continuous professional development and contribute to the training of pharmacy staff and students.

Qualifications

Essential:

  • Master's Degree in Pharmacy or equivalent.
  • Registered member of the relevant professional body.
  • Post-graduate qualification in Clinical Pharmacy.

Desirable:

  • Independent prescriber status.

Experience

Essential:

  • A minimum of three years' experience as a hospital pharmacist.

Desirable:

  • Experience in providing pharmaceutical care to palliative care patients.
  • Teaching experience.
  • Experience in developing prescribing policies.
