About the Role

Heathview Medical Practice is seeking a highly skilled Clinical Pharmacist to join our team. As a Clinical Pharmacist, you will play a vital role in providing direct clinical care to patients, working closely with our GP and nursing staff to deliver high-quality patient care.

Key Responsibilities

  • Participate in all treatment and preventative healthcare services in the practice, as delegated and agreed by the supervising GP.
  • Provide direct clinical care to patients using established clinical guidelines.
  • Recommend and explain appropriate diagnostic tests and treatment.
  • Discuss patients with problems needing referral to secondary care with the supervising GP before making such referral.
  • Ensure evidenced-based care is delivered at the highest standards, ensuring delivery of high-quality patient care.
  • Establish and maintain good liaison with other surgeries and agencies, including secondary care.
  • Undertake face-to-face and telephone consultations for emergency or routine problems, as determined by the practice, including management of patients with long-term conditions.
  • Contribute positively to the effectiveness and efficiency of the team and work colleagues.

  • Qualifications
    • Essential:
      • Registered with the GPHC.
      • Educated to degree/masters level in Pharmacy, MPharm/BPharm (or equivalent).
  • Personal Qualities
    • Essential:
      • Excellent planning and organisation skills.

      • Motivated and proactive.
      • Forward thinker.
