Health Promotion Specialist

We are seeking a highly skilled and motivated Health Promotion Specialist to join our team at Homerton Healthcare NHS Foundation Trust. As a Health Promotion Specialist, you will play a key role in promoting the health and wellbeing of looked after children and care leavers.
Key Responsibilities- Develop and implement health promotion strategies to address the health needs of looked after children and care leavers.
- Work closely with the Looked After Children Nurses to identify and address health promotion needs.
- Deliver health promotion interventions to children, young people, and care leavers on a one-to-one and group basis.
- Facilitate the Health Improvement Agenda for Looked after children and support the delivery of activities to promote health and wellbeing.
- Monitor and evaluate the effectiveness of health promotion interventions.
- Develop and maintain relationships with partner agencies to promote health and wellbeing.
- Registered Nurse with a degree in a relevant discipline.
- Experience of working in health promotion or a related field.
- Excellent communication and interpersonal skills.
- Ability to work autonomously and as part of a team.
- Knowledge of health promotion principles and practices.
