About the Role:

We are seeking a skilled and compassionate Complex Needs Practitioner to join our team. As a key member of our Transitions team, you will work with young people aged 17-25 who are affected by substance misuse, including those impacted by their parents' substance misuse.

Your primary focus will be to engage young people in various interventions, using a young person-centered approach to empower them to reduce risk, increase resilience, and achieve positive outcomes.

Key Responsibilities:
  • Work with young people and their families to address holistic support needs across all aspects of their wellbeing.
  • Develop and implement individualized plans to support young people in achieving their goals.
  • Collaborate with multi-agency partners, including health, social care, education, and criminal justice, to ensure a comprehensive approach to support.
  • Provide guidance and support to young people on managing risk, increasing resilience, and achieving positive outcomes.
Requirements:

We are looking for an energetic and confident self-starter with proven experience in health and social care, particularly working with young people and supporting them through mental ill health.

A strong understanding of the complexities of substance misuse and its impact on young people and their families is essential.
