Job Summary:

We are seeking a highly skilled and dedicated Community Respiratory Specialist to join our team at West London NHS Trust. As a Community Respiratory Specialist, you will play a vital role in supporting chronic respiratory patients in the community, providing them with the necessary support to self-manage their conditions.

Key Responsibilities:

  • To contribute to the care provision and review of respiratory patients, thereby minimizing risks and promoting recovery by supporting a multi-disciplinary approach to delivering individualized care, self-management, and preventing unplanned hospital admissions.
  • To empower clients, adopting a problem-solving approach, encouraging and giving clients the confidence and skills to improve the self-management of their chronic respiratory disease.
  • To work as part of a dynamic multi-disciplinary team comprising nurses, physiotherapists, therapist assistants, and administrative staff to provide high-quality care to patients.
  • To support the development and implementation of individualized care plans, taking into account the unique needs and goals of each patient.
