We are seeking an experienced Care Coordinator to manage essential administrative tasks associated with care delivery. This role is primarily office-based and involves responsibilities such as managing client files, generating compliance documentation, and occasionally being available for on-call duties as scheduled. You will collaborate with healthcare professionals, care staff, clients, and their families to uphold high standards of care.

Reporting To: Care Manager

Role Summary:

The primary objective is to ensure the effective allocation of care visits, recruit qualified Care Assistants, and uphold efficient office procedures.

Main Responsibilities:

  • Responding to all incoming inquiries via telephone.
  • Implementing comprehensive recruitment and vetting processes for Care Assistants.
  • Processing new client referrals efficiently.
  • Preparing necessary documentation for Field Care Supervisors.
  • Ensuring consistent allocation of Care Assistants to client visits, guaranteeing coverage at all times.
  • Managing all Individual Care & Support Agreements.
  • Maintaining up-to-date client and Care Assistant files.
  • Keeping the computer system current at all times.
  • Ensuring all documentation and electronic records for clients and Care Assistants are accurate and up to date.
  • Maintaining all other relevant records accurately.
  • Collaborating closely with the Field Care Supervisor.
  • Ensuring compliance with all office systems and procedures.
  • Assisting with payroll and invoice processing.
  • Generating reports and management information as required.
