Our Virtual Wards facilitate the delivery of necessary care within patients' homes, including care homes, ensuring safety and convenience. This model also addresses the disparity between healthcare demand and the availability of secondary care beds by offering alternatives to hospital admissions and promoting early discharges.
Supported by a robust evidence base, Virtual Wards yield numerous benefits for patients, healthcare systems, and the public. These advantages include increased patient autonomy and personalized care, the ability for patients to receive treatment in their home environment, reduced emergency department visits and hospital admissions, and improved staff experiences through flexible working arrangements and blended roles that combine in-person and virtual care.
Key Responsibilities
We are seeking Community Nursing Associates to join our dedicated Community Team, providing care directly in patients' homes and care facilities. You will benefit from a comprehensive induction program and ongoing clinical supervision, along with opportunities for professional growth and development within the team. Additionally, you will play a role in advancing technology-enabled local systems.
The service operates seven days a week, ensuring a minimum coverage period and incorporating a multidisciplinary team with diverse clinical expertise to guarantee equitable care across the entire service area, including an overnight nursing service. A thorough clinical triage process will be in place to accept suitable referrals from various partner organizations through established clinical pathways.
Applicants should possess demonstrable post-registration experience in delivering complex care within a community context, effective implementation of clinical governance, and evidence of ongoing professional development relevant to this clinical field at the Nursing Associate level. You should also provide proof of nursing practice skills and demonstrate the impact of these skills on practice improvement and clinical service development.

Qualifications and Knowledge
Essential
- Registered Nursing Associate on the NMC register
- Understanding of NMC professional Revalidation requirements
- Proficient IT skills for email, online staff records, clinical ordering systems, and patient record systems
- Solid understanding of Community Nursing principles
- Awareness of the Nursing Associate's role within the interdisciplinary team and organization
- Comprehension of the importance of adhering to procedures and care plans
Desirable
- Evidence of continued professional development (CPD)
- Experience in safely administering prescribed medications
- Understanding or application of relevant clinical practices, standards, and audits
- Broad knowledge of relevant policies and legislation and their application to clinical and service areas
