Position Overview

Preston Park Community PCN Limited Company is seeking to appoint an Advanced Clinical Practice Nurse to enhance our multi-disciplinary team. This role is pivotal in modernizing care pathways and facilitating the effective sharing of expertise across various professional domains.

The ideal candidate will possess a diverse professional background. As a seasoned healthcare practitioner with a master's level education, you will have honed the necessary skills and knowledge to undertake an expanded role in patient care.

Key Responsibilities

  1. Exhibit a high degree of proficiency within your specialized service, offering guidance, education, and support to colleagues, patients, and their families.
  2. Function autonomously as an Advanced Clinical Practice Nurse within your designated specialty, delivering patient-centered clinical care. This includes advanced clinical assessment, examination, diagnosis, and treatment within an established scope of practice.
  3. Champion innovative working methods that prioritize efficiency and patient-centered services, ensuring the safe treatment, referral, and discharge of patients presenting with undiagnosed conditions.
  4. Lead the continuous enhancement of clinical practices and care standards within the service, collaborating with multidisciplinary colleagues to develop policies, procedures, protocols, and guidelines.

Essential Qualifications and Skills

  • Proven ability to foster effective working relationships and promote collaborative practices with both internal and external stakeholders.
  • Possession of a clean driving license and access to a vehicle with appropriate business insurance.
  • Current registration with the Nursing and Midwifery Council, with up-to-date mandatory training and revalidation.
  • A postgraduate qualification at level 7 or above relevant to your area of enhanced practice, such as in wound care, diabetes, respiratory and cardiovascular health, dementia, women's health, or public health.
  • Strong verbal and written communication skills, adaptable to a variety of audiences.
  • Proficient in ICT with good organizational and time management skills.
  • Professional demeanor, exhibiting calmness and efficiency.
  • Self-motivated, capable of working independently as well as collaboratively within a team.
  • Ability to work safely and effectively both autonomously and in conjunction with other healthcare professionals.

Desirable Qualifications and Experience

  • Evidence of ongoing professional development post-qualification.
  • Experience in a General Practice setting.
  • Familiarity with System 1 and EMIS.
  • Understanding of national and local healthcare guidelines.
