Overview:
An exceptional opportunity has emerged for a Senior Complex Care Coordinator to collaborate with a multidisciplinary team of healthcare, social services, and independent sector professionals. This role focuses on comprehensive assessments for adults over 18 years with intricate health requirements.
Key Responsibilities:
- Conduct thorough assessments, reviews, and monitoring of care for individuals whose health needs are funded through NHS Continuing Healthcare within specified timelines.
- Serve as a Case Manager for clients within the integrated care framework, evaluating the needs of clients and their families to facilitate, plan, and assess service delivery, ensuring optimal functioning and appropriate care in suitable settings.
Qualifications:
- Dual registration as a Learning Disabilities Nurse/Registered Nurse with current NMC registration or equivalent.
Essential Experience:
- Proven experience in conducting Continuing Care Assessments.
- Extensive background working with adults and young individuals with complex, long-term health conditions, disabilities, and those requiring palliative or end-of-life care.
- Demonstrated leadership, management, and team development skills.
- Understanding of the transition processes for youth moving into adult care.
- Exceptional verbal and written communication skills, capable of presenting complex cases effectively.
- Strong analytical skills to identify risks, foresee challenges, and devise solutions related to service delivery.
- Ability to travel as necessary, with a valid driver's license or suitable alternative transportation.
Assessment Standards:
- Ensure that assessments are thorough, transparent, and involve multiple agencies when necessary for NHS Continuing Healthcare/NHS-funded Nursing Care.
Additional Responsibilities:
- Analyze supporting documentation to establish a comprehensive history of the patient's needs, leading to a specialized nursing assessment.
- Conduct NHS-funded Nursing Care (FNC) evaluations for service users requiring ongoing nursing care in residential settings, ensuring compliance with current guidelines and regular reviews.
- Support the implementation of personal health budgets for those assessed as needing them.
