The Commonwealth of Virginia is seeking a highly skilled Population Health Manager to join our team. As a key member of our management team, you will be responsible for planning, implementing, and evaluating health improvement activities in our local health district.

Key Responsibilities

  • Develop and implement health improvement plans to address the needs of our community
  • Collaborate with stakeholders to identify and prioritize health improvement initiatives
  • Conduct needs assessments and analyze data to inform health improvement strategies
  • Develop and manage grants to support health improvement projects
  • Provide technical assistance and training to local and community agencies
  • Serve as a public contact and liaison for health improvement activities

Requirements

  • Considerable knowledge and experience in health education, community health assessment, and improvement planning
  • Experience in project management, grant writing, and data analysis
  • Excellent communication and interpersonal skills
  • Proficiency in Microsoft Office products

Preferred Qualifications

  • Previous experience in public health and health improvement planning
