Job Summary
The Commonwealth of Virginia is seeking a highly skilled Population Health Manager to join our team. As a key member of our management team, you will be responsible for planning, implementing, and evaluating health improvement activities in our local health district.
Key Responsibilities
- Develop and implement health improvement plans to address the needs of our community
- Collaborate with stakeholders to identify and prioritize health improvement initiatives
- Conduct needs assessments and analyze data to inform health improvement strategies
- Develop and manage grants to support health improvement projects
- Provide technical assistance and training to local and community agencies
- Serve as a public contact and liaison for health improvement activities
Requirements
- Considerable knowledge and experience in health education, community health assessment, and improvement planning
- Experience in project management, grant writing, and data analysis
- Excellent communication and interpersonal skills
- Proficiency in Microsoft Office products
Preferred Qualifications
- Previous experience in public health and health improvement planning
