Job Summary

The Commonwealth of Virginia is seeking a highly skilled Population Health Manager to join our team. As a key member of our Management Team, you will play a critical role in planning, implementing, and evaluating health improvement activities in the local health district.

Key Responsibilities
  • Develop and implement health improvement plans to address the needs of the local health district
  • Collaborate with stakeholders to identify and prioritize health improvement initiatives
  • Conduct needs assessments and facilitate the CHIP process to inform health improvement planning
  • Prepare and submit grant applications to secure funding for health improvement projects
  • Deliver educational programs to clients and staff, and conduct health education trainings
  • Develop and revise educational materials to support health improvement initiatives
  • Collect and analyze program-specific data to inform health improvement planning
  • Provide technical assistance to local and community agencies/organizations on program planning
  • Serve as a public contact and PIO for services and activities
Requirements
  • Considerable knowledge and experience in health education, community health assessment, and improvement planning
  • Experience performing research and utilizing quantitative and qualitative methods for assessing data and programs
  • Experience with project management, grant management, and grant writing
  • Excellent communication skills, verbally and in writing, to effectively manage interpersonal and interorganizational communication and coordination networks
Preferred Qualifications
  • Previous experience in public health and the assessment/improvement process
