About the Role

We are seeking a highly skilled and dedicated Community Nurse to join our team at Sussex Community NHS Foundation Trust. As a Community Nurse, you will play a vital role in delivering high-quality care to patients in their own homes and community settings.

Key Responsibilities
  • Deliver comprehensive care to patients with complex needs, including wound management, catheterization, and palliative care.
  • Work closely with multidisciplinary teams to ensure seamless care coordination and patient outcomes.
  • Develop and maintain strong relationships with patients, families, and carers to promote independence and choice.
  • Contribute to the development and implementation of care plans, ensuring patient-centered and evidence-based practice.
  • Participate in clinical governance, audit, and supervision to maintain high standards of care and professional development.
Requirements
  • Registered Nurse qualification and ENB 998 or equivalent.
  • Experience in a community setting, with a strong understanding of community health and social care services.
  • Excellent clinical skills, including wound management, catheterization, and palliative care.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues.
  • Ability to reflect and learn from situations, with a commitment to lifelong learning and development.
