About the Role

We are seeking a highly skilled and dedicated Community Nurse to join our team at Sussex Community NHS Foundation Trust. As a Community Nurse, you will play a vital role in delivering high-quality care to patients in their own homes and community settings.

Key Responsibilities
  • Deliver comprehensive care to patients with a range of health conditions, including diabetes, wound management, and palliative care.
  • Work closely with patients, families, and carers to develop and implement personalized care plans.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and communication.
  • Conduct regular assessments and reviews to monitor patient progress and adjust care plans as needed.
  • Provide education and support to patients and their families to promote independence and self-management.
Requirements
  • Registered Nurse qualification and current registration with the NMC.
  • Experience in a community setting, preferably with a focus on diabetes, wound management, and palliative care.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a flexible approach to working hours and locations.
  • Knowledge of current local policies and guidelines informing community health and social care services.
