About the Role

We are seeking an experienced Band 6 registered nurse to join our multidisciplinary community team in the Ryedale/Scarborough locality. The successful applicant will support service development and patient care within our Diabetes service.

Key Responsibilities
  1. To support the delivery of a Community Diabetes Service that adheres to the National Service Framework for Diabetes.
  2. To hold a caseload for the management of patients with complex diabetes presentations who require specialist assessment and treatment.
  3. To deliver structured education programmes to patients and clinical support to health and social care.

Person Specification

Qualifications and Knowledge

  1. Registered Adult Nurse on NMC live register.
  2. Post registration/graduate diploma or equivalent experience in related work area, to demonstrate advanced level of practice.
  3. Experience of Clinical governance & risk management.

Experience

  1. Minimum of 2 years' experience at Band 6.
  2. Solid base of experience in diabetes care.
  3. Commitment to patient-focused care.
