Diabetes Specialist Nurse

We are seeking an experienced Registered Nurse to join our multidisciplinary community teams in the Scarborough and Ryedale locality. The successful candidate will support service development and patient care within the Diabetes service.
Main Responsibilities- Support the delivery of a Community Diabetes Service that aligns with the National Service Framework for Diabetes recommendations.
- Manage patients with complex diabetes presentations, providing care and support for secondary care and primary diabetes community service, including joint working with diabetes dietitians and consultants.
- Deliver structured education programs to patients and clinical support to health and social care professionals.
- Registered Adult Nurse on the NMC live register.
- Post-registration/graduate diploma or equivalent experience in a related work area, demonstrating advanced practice.
- Evidence of ongoing Continuing Professional Development (CPD).
- Presentation and teaching skills.
- Experience of Clinical Governance and Risk Management.
- Minimum of 2 years' experience at Band 5.
- Solid base of experience in diabetes care.
- Ability to work independently and within a multidisciplinary team.
- Commitment to patient-focused care.
