Job Summary
The Occupational Therapist will work as part of an Integrated Multidisciplinary Team to support community reablement, prevent unnecessary admissions to hospital or long-term care, and facilitate timely discharge from hospital.
Key Responsibilities
- Carry out functional, holistic, and specialized assessments to support individuals in maximizing their independence.
- Develop and implement interventions to address barriers to independence, including recommendations for minor and major adaptations to clients' homes.
- Work collaboratively with Health and Social Care colleagues to provide a comprehensive service to individuals and their families.
- Supervise junior staff and support student placements to ensure the development of a skilled and knowledgeable team.
- Contribute to service development, maintain professional standards, and governance to ensure the delivery of high-quality services.
- Travel to various locations within the community to provide services and participate in a seven-day working roster.
Requirements
- Hold a varied caseload and work effectively as part of a multidisciplinary team.
- Be highly motivated, enthusiastic, flexible, and resilient to meet the demands of the role.
- Have the ability to speak Welsh is desirable, but English and/or Welsh speakers are equally welcome to apply.
